Employee Benefits

Lankota employees are entitled to a wide range of benefits. A number of the benefit programs -- such as Social Security, workers' compensation, state disability and unemployment insurance -- cover all employees as required by law.

Eligibility for most other benefits depends upon a variety of factors, including employee classification. Your supervisor can identify the programs for which you are eligible. You can find the details of many of these programs in separate written summaries. For more information, contact Human Resources.

Some benefit programs require contributions from employees, but Lankota contributes some financial responsibility towards a few benefits. We reserve the right to add, amend, modify or terminate any employee benefit plans or programs.

If an employee is not enrolled in the medical or dental benefit program there is an open enrollment month for each when an employee can add or change paid benefits for the coming year. Please refer to your Human Resource Manager for these open enrollment months.

Medical Insurance
Lankota offers Wellmark Blue Cross Blue Shield insurance and bears a substantial cost of this plan for the employee. This is available after the full sixty (60) day probationary period is completed. Refer to the printed information from the insurance provider for details of eligibility and coverage or contact the Human Resources Manager.

COBRA
If you are terminated for any reason other than gross misconduct or otherwise leave Lankota and have been continuously covered by Lankota’s group insurance plan before that, you and your dependents may have the right to continue or convert coverage as set forth in the rules of the plan. You should receive written notification of your right to continue coverage within two weeks of your last day on the job. Should you have questions about this coverage, contact Human Resources.

Retirement Saving Plan
Lankota provides a simple plan for retirement for fulltime employees who have completed a full year of service with the company and otherwise qualify to participate. Lankota will match up to 3%.

The plan trustee is Cornerstone Financial Solutions, Inc.

Supplemental Insurance
Supplemental insurance such as vision, accident, cancer, short term disablitiy, etc. is offered through Colonial Life after the full sixty (60) day probationary period is completed. Our Colonial Life agent will come to the premises to meet with you. These deductions will be taken out of your payroll check and the premium will then be paid by Lankota. Please refer all questions to your Human Resources Manager.

Dental Insurance
Lankota pays 25% per month towards single/family coverage through Delta Dental (the cost fluctuates). You must work the full sixty (60) day probationary period to be eligible for dental insurance. This deduction will be taken out of your payroll check and the premium will then be paid by Lankota. Please refer all questions to your Human Resource Manager.

Worker's Compensation
Lankota provides insurance to compensate for any illness or injury an employee might suffer while working on company premises, traveling on official company business, or attending an activity officially sponsored by Lankota. If you become ill or injured, please get medical attention at once.

You must also report the details to your supervisor immediately. And you must complete a report for every injury, no matter how small, to keep the coverage in force and to get any benefits or other compensation to which you may be entitled.

Long Term/Short Term Disability
Lankota pays 100% of the premium for each employee to cover Long Term/Short Term Disability. This will cover injuries that sustained on the work premises or off the work premises. Please refer any questions to your Human Resource Manager.
